Description

NHS Central London Clinical Commissioning Group (the Commissioner), acting as Lead Commissioner on behalf of itself, Hammersmith & Fulham Clinical Commissioning Group and West London Clinical Commissioning Group, together with London Borough of Hammersmith and Fulham, Westminster City Council and Royal Borough of Kensington & Chelsea are commissioning an integrated Community Independence Service (CIS)

The service areas included within this opportunity are as follows:
• Rapid response
• In-reach/ supported discharge
• Rehabilitation
• Reablement

The above services are part of a number of intermediate care services available to patients, currently being delivered by various providers. NHS England has made a commitment to developing a coherent 24/7 urgent care service in every area of England that makes sense to patients when having to make choices about care and to drive the integration of services. It also committed to the provision of integrated NHS services seven days a week in order that patients experience genuine seven-day care.
It is expected that better integration will be delivered through: - Improved communications within and between service providers; - Improved coordination of services; - Reduced duplication of services and activities.

Interested parties are now invited to express their interest and download the Invitation to Tender (ITT) documentation.

The CCG anticipates awarding a contract for up to 21 months. The expected service commencement date is 11th July 2016. The total value of the contract over 21 months is estimated at £31,000,000.
