Description

Invite to provider engagement event

NHS Nottingham City, Mansfield and Ashfield, Newark and Sherwood, Nottingham North and East, Nottingham West and Rushcliffe Clinical Commissioning Group are establishing a list of Approved Providers who can provide packages of care in the community to children and young people who meet the threshold for children's continuing care to support in accordance with the specification.

This framework will be used to support all children and young people eligible for Childrens Continuing Care across the City and 6 County CCGs.
This framework is open to any homecare provider who can demonstrate they can meet the health needs of children and young people with high level complex health and disability who are in receipt of Childrens Continuing Care.

The procurement main aims are:
• To establish a list of Approved Providers, who can deliver high quality, outcome focused packages of care for children and young people living at
home with their families.
• To develop a service which can meet the needs of children, young people and their families in Nottingham City in accordance with the principles of the National Framework for Children and Young People's continuing Care guidance and the national personalization agenda.
• Consider a Framework of Providers who will offer flexible services that will respond to the changing needs of Children and Young people based on a whole life model of health and social care requirements.
• Consider how a Framework of Providers will also support the implementation of personal health budgets across health and social care services.
