Description
Rotherham Clinical Commissioning Group (CCG) (the Commissioner) wishes to establish a panel of qualified providers (the Provider) who can provide consistently high quality, domiciliary care services to adults aged 18 and over who have been assessed as eligible for NHS Continuing Healthcare. The Services being commissioned under this accreditation process will be procured via the Any Qualified Service Provider (AQP) model. The procurement will be broken down into two lots:
Lot 1 - Basic Domiciliary Care for Continuing Healthcare Patients to assist with their daily living needs, and;
Lot 2 - End of Life Care for those patients who have been fast tracked into the service or transferred from continuing care.
