Description
Derbyshire County Council, in conjunction with local partners the National Probation Service (NPS), Safer Derbyshire Community Safety Partnership, and the Office of the Police and Crime Commissioner, is seeking to commission a service provider to deliver a health trainer service to adult offenders on community orders or licences delivered by Derbyshire Probation Services for the areas covered by the Chesterfield office teams (map attached as appendix). A snapshot audit in October 2020 identified that the Chesterfield offender cohort consists of 736 clients, with 524 managed by the Community Rehabilitation Company (CRC), and 212 managed by the NPS, of which 84 are deemed to be a high risk of causing harm.
Health Trainer services support adults who wish to make a change towards a healthier lifestyle but who are unlikely to make and maintain this without individual support. Probation Health Trainers are generally ex-offenders, and all health trainers must have successfully completed the nationally accredited Health Trainer training and been assessed as competent to undertake the Health Trainer role. Health Trainers work with clients on a one-to-one basis to assess their health and lifestyle risks and support them in their decision to make and sustain behavioural change to a healthier lifestyle.
A full evaluation of the service will be required to assess whether the programme achieved the outcomes and how the service could be delivered sustainably across Derbyshire.
The service will commence on 01 August 2021 with an end date of 31 July 2023
