Description

The partner organisations, Health and Education, are jointly seeking a provider who can provide the services of a Care Co-ordinator for co-ordinating all the health and education services provided for a young person as specified in the Education, Health and Care Plan.

The care co-ordinator will be a qualified health professional with experience of autism and attachment- based difficulties and learning difficulties

The role will undertake the following primary activity

- One weekly session of one- hour duration - to ensure consistency and liaison between services and therapists and family.

- An initial preparation session to include report reading and liaison with professionals and family. This will be for a minimum of one day duration and will be in addition to the weekly session.

- Organise and attend half termly professionals' meetings and provide an annual written progress report and attend the EHCP annual review

- This role will work closely with the Local Authority and the post 16 education provider where some of the health services are delivered, alongside universal health services outside the educational setting.

- The Services to be co-ordinated include all health provision some of which may be delivered through College, Universal Health Services, Psychology, Mental health universal services, Primary care GP, Social Care and Education provision

The co-ordination role is expected to include:

- Attendance at all Multi-Disciplinary Teams (MDT) in all settings and all educational progress review meetings
- Co-ordination of multi-agency services provided to a young person to ensure consistency

- Involvement in relevant multi-agency meetings and education reviews.

- Liaison with the YP, their family and involved agencies and professionals


The service is intended to last for approximately 39 weeks mirroring the academic year with a fixed price per hour. Commissioners are looking for quotations from suitably qualified and competent providers, to deliver a high quality service. Commissioners are aiming to award and service to start as soon as possible on a date to be agreed.
