Structured upskilling of the Trusts' internal data and digital\r\nteams.\r\nPhase 4: Embedding, Handover and Legacy (Months 10-12): Production of a full product\r\nhandover and Champion Trust End-of-Year Outcomes Report; knowledge transfer; finalisation of\r\nthe Transformation Roadmap of the past 12 months.",
