The aim is to deliver positive outcomes, improve lives and prevent and reduce crisis by offering local community-based activities wherever possible.\r\n The Trust will soon be commissioning Targeted Short Break Services for children and young people in the City who do not require a social work assessment, but who meet other eligibility criteria-such as having an Education, Health and Care Plan (EHCP), receiving Disability Living Allowance (DLA), or having a formal diagnosis.",
