Description
The L&Q Foundation are looking to introducing an outreach service to our residents where by a diagnostic assessment can be completed either within the resident's home or locally. With a person-centred approach, we are looking for an organisation to use its experience and skills to help encourage residents to take up opportunities either locally to them or via the provider themselves remotely.
We are looking for a provider who has a track record in this type of delivery, understands this customer base, can provide a person-centred approach whilst encouraging residents to access services that will better their health.
The initial programme will run for 24 months, engaging with at least 300 residents per year with at least 75% engaging in some form of H&W initiative and at least half continuing to be engaged in the programme for at least 4 weeks.
As part of our market testing we would like to speak to organisations, including consortiums, which can deliver this programme across South London, North & North-East London. Specific boroughs in which we would be looking to run a programme are Lewisham, Southwark, Haringey, Hackney and Barking and Dagenham and Slough.
These discussions will not constitute any form of commitment on behalf of L&Q or the provider(s) who take part. The deadline for expressions of interest are the 21st December 2018.
